Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates assigned patients and develops, implements, and modifies treatment plans that align with the patients' needs, goals, and physician direction. The therapist is responsible for the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring a functional treatment program for effective community reintegration.

Responsibilities

• Complete a comprehensive evaluation of assigned patients.
• Utilize clinical reasoning to create appropriate treatment plans focused on patient progress.
• Document medical records accurately and timely in compliance with regulations and policies.
• Participate in the student program, providing mentorship and evaluations.
• Actively pursue professional development through continuing education and professional activities.
• Adhere to the discipline-specific State Practice Act and maintain professional licensure in good standing.
• Demonstrate commitment to safety and adherence to workplace policies to ensure a safe environment for patients and staff.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• Minimum of 1 year of outpatient clinical experience.
• Current CPR certification.

Schedule

This position follows a 5x8 schedule, offering 40 hours per week, Monday through Friday, starting in early July.
