Schedule/Hours:

• 36-hour guaranteed week (if eligible), weekend coverage may be required
• Flexibility to travel across the broader Alameda County area as needed

Key Qualifications:

• Active California Physical Therapy License
• Current BLS certification
• Valid driver's license, auto insurance, and registration
• Annual TB test, up-to-date MMR, Varicella, HepB, Tdap (within 10 years), annual flu shot
• Completed drug screen and physical assessment

Core Responsibilities:

• Deliver high-quality PT services in a home health setting, managing a diverse caseload
• Perform comprehensive evaluations (OASIS, routine, discharge, SOC, and follow-up)
• Document patient progress efficiently and securely
• Communicate effectively with patients, families, and interdisciplinary providers
• Adapt to dynamic scheduling and territory assignments based on patient needs
