Duties / Responsibilities Senior Social Workers in the Primary Care at Home program play a critical role in supporting patients and families by identifying social, emotional, and environmental needs and connecting them with appropriate resources and services. This role provides a full spectrum of social work and discharge planning services, delivering comprehensive case management for patients with complex medical, geriatric, or behavioral health needs. As a key member of the care team, the Senior Social Worker helps ensure patients receive coordinated, compassionate support that promotes stability, independence, and overall well‑being. Position Qualifications Master’s degree in social work (MSW) from an accredited graduate school of Social Work Minimum of 1-2 years of related work experience preferred. Hourly Pay Range Hourly Pay Range: $30.34 - $48.55 This pay rate/range represents ChristianaCare’s good faith and reasonable estimate of compensation at the time of posting.
