The role supports patients in New Kensington and surrounding areas, providing high‑quality care that has a significant daily impact. Responsibilities Collaborate with the care team to develop and implement comprehensive patient care plans. Provide direct bedside care and support to hospice patients and families. Maintain accurate and timely charting, meeting all critical documentation deadlines. Administer medications, treatments, and symptom management as per physician orders. Educate patients and families on disease progression, comfort measures, and available resources. Coordinate with interdisciplinary team members to ensure continuity of care. Conduct home health visits within a 20% travel requirement of patient locations. Utilize tablets and mobile EMR technology for efficient documentation and communication. Contribute to a supportive environment by participating in mentorship and professional development programs. Adhere to safety, infection control, and regulatory standards in a home environment. Qualifications RN license in the state of employment. Associate degree or higher from an accredited nursing school. Minimum of two years of RN experience; hospice experience preferred. Valid driver’s license with ability to travel ~20% of work hours to patient sites. Proficiency with tablets, smartphones, and electronic medical record systems. Strong time‑management and self‑management skills. Commitment to meeting critical charting deadlines.
