• Facility Specific Job Notes:

**Required Experience and Qualifications:** - 1 year of previous experience - At least 1 travel contract completed - Epic experience - Siemens Edge equipment experience - Iowa Permit to Practice - State license - Not an employee of UnityPoint in the last 12 months - Driver's License - Work history/resume - Skills checklist completed within 1 year of submission date - License verification - References - 1 supervisory reference required at the time of submission - Must have a taxable address outside of 50 miles from the facility, unless in Madison with a 10% rate reduction **Preferred Certifications and Licensure:** - RT(R)(CT) **Facility Information:** - Madison, Sioux City, Bettendorf, and Fort Dodge are level 3 trauma centers - Moline is a level 2 trauma center - Muscatine is a level 4 trauma center - Rock Island is a level 1 trauma center
