Work Environment

The role is entirely onsite and dedicated to concierge medical care, providing house calls and personalized clinical services directly to patients. This distinct model ensures our Nurse Practitioners can deliver flexible, patient-centered care while building strong relationships with the women we serve. You will work in a collaborative setting, supporting both in-office appointments and at-home visits, which broadens access to healthcare for our clients.

Key Responsibilities

• Conduct thorough initial patient histories and perform comprehensive physical examinations to assess health conditions accurately.
• Develop, document, and communicate individualized care plans, ensuring to follow up with patients to monitor their health and adherence to treatment.
• Perform daily patient rounds, assessing the condition of patients under your care and adjusting treatment plans as necessary.
• Utilize advanced health technologies to write and coordinate orders for medications, laboratory work, and diagnostic tests.
• Interpret laboratory results and diagnostic tests to make informed decisions about patient care.
• Collaborate with physicians, nursing staff, and interdisciplinary care teams to maintain high standards of patient care.
• Document and communicate discharge summaries, prescriptions, and post-discharge care instructions professionally and clearly.
• Participate in professional development and continuing education activities to enhance clinical skills.

Patient Population / Client Focus

This position focuses on a diverse patient population, including women across various life stages and health conditions. As a Nurse Practitioner, you will address a wide range of health issues, from gynecological and reproductive health to chronic conditions and preventative care, ensuring that each woman receives personalized attention that encompasses their unique health journey.

Qualifications

• Master’s Degree from an accredited Nurse Practitioner Program with a specialization in women’s health.
• National Board Certification in a relevant specialty area is required.
• Current licenses to practice as a Nurse Practitioner and as a Registered Professional Nurse in New York State.
• Possession of a collaborative practice agreement with appropriate protocols filed with the NYS Department of Education or equivalent as required by the Nurse Practitioner Modernization Act of 2015.

Preferred Experience

A minimum of one year of experience as a Nurse Practitioner, specifically in women's health, is preferred. Familiarity with medical technologies and EMR systems is advantageous to facilitate efficient patient care processes.
