Registered Nurse Hospice Case Manager

Registered Nurse Hospice Case Manager at BJC HealthCare will coordinate and deliver compassionate end-of-life care for a dedicated caseload of patients in home and facility settings. Responsibilities include comprehensive assessments, pain and symptom management, and development of individualized care plans. The RN collaborates with an interdisciplinary team, educates families, and ensures high-quality, ethical care consistent with BJC’s mission of safety, excellence, and innovation in a supportive, collaborative environment focused on patient outcomes.
