Physical Therapist

Evaluates and assesses the client based on subjective and objective information obtained through medical record review, formal and informal test administration with appropriate analysis and interpretation of results to develop the plan of care. Includes OASIS assessment at appropriate time points. Formulates written functional measurable goals, treatment plans, and home programs for clients. Identifies barriers to describe outcomes. Evaluates equipment needs to increase function level. Plans for discharge. Provides treatment according to established goals on the treatment plan and reassesses the client's condition. Revises the plan of care in consideration with the physician and other care-team members. Evaluates the effectiveness and outcome of care. Instructs/educates clients/caregivers in appropriate activities and referrals to community resources as appropriate utilizing both verbal and written programs. Documents evaluations and progress in the clinical record and maintains client record as required by the agency director. Communicates with clients, caregivers, and all health care professionals required to provide comprehensive interdisciplinary care. Attends case conferences at the agency when physical therapy clients are discussed. Maintains skills and knowledge through training and continuing education to provide expected standards of care.
