Advanced Heart Failure APP Opportunity

This is an exceptional opportunity for a motivated NP/PA to join an employed position within a busy, well-respected practice, supported by a strong referral base and the full resources of a robust health system network.

The Advanced Heart Failure APP will work as part of a multidisciplinary team providing comprehensive care to patients with heart failure, including those receiving guideline-directed medical therapy (GDMT) and advanced heart failure therapies. This role primarily involves close collaboration with cardiologists, nurses, pharmacists, and care management teams to optimize patient outcomes and reduce hospitalizations.

• Provide comprehensive evaluation and management of heart failure patients in the outpatient/inpatient setting.
• Optimize and titrate guideline-directed medical therapy in accordance with current heart failure guidelines.
• Coordinate transitions of care and timely follow-up following hospitalizations.
• Collaborate with physicians and multidisciplinary team members to develop individualized treatment plans.
• Educate patients and caregivers on disease management, medications, diet, and lifestyle modifications.
• Monitor patient progress, review diagnostic testing, and adjust treatment plans as appropriate.
• Participate in quality improvement initiatives and evidence-based practice efforts within the Heart Failure Program.
• Maintain accurate and timely documentation and support program quality metrics.
• Provide inpatient heart failure rounding coverage every fourth weekend to support continuity of care.
• Prior Cardiology and/or Heart Failure experience preferred
• Board Certified Nurse Practitioner or Physician Assistant
• Eligible for state medical license and DEA/Controlled Substance registration
• BLS and ACLS certification required

Schedule:

• Nine half-day outpatient clinic sessions per week.
• Inpatient heart failure rounding every fourth weekend.

What Minimum Requirements You'll Need

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Nurse Practitioner credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date required.
• Drug Enforcement Admin credentialed from the Drug Enforcement Administration preferred.

Education:

• Master's degree of Nursing required.
