Care Manager RN

Care Manager RN at Providence Little Company of Mary Medical Center San Pedro in San Pedro, CA. This position is Per Diem and will work 8-hour, Day shifts.

The case manager performs the primary functions of assessment, planning, facilitation and advocacy, which are achieved through collaboration with the client and other health care professionals involved in the client's care.

"Case management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual's and family's comprehensive health needs through communication and available resources to promote quality cost effective outcomes." Required Qualifications:

• Bachelor's Degree
• Upon hire: California Registered Nurse License
• 2 years Acute hospital experience, including one year of supervisory experience.

Preferred Qualifications:

• Master's Degree In health care related field.

Posted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
