Growing together.The Associate Patient Care Coordinator is responsible for the completion of set processes and protocols. Works cooperatively with all members of the care team to support the vision and mission of the organization, deliver excellent customer service and adhere to Lean processes. Supports the teams in meeting financial, clinical and service goals.Primary Responsibilities:Call patients to remind them of scheduled appointmentsSupport the Clinical team in chart preparation and request medical informationExplain the process of the wellness visit to patients including, but not limited to what is involved, who the provider is, and the location of the specialist office. Ability to directly schedule patients into all listed clinics to ensure a seamless transition for patientsResponsible for obtaining 'expert' level knowledge of the inter-workings of the Medicare Advantage plansResponds effectively to incoming patient calls and communicates pertinent information to designee to provide for continuity and optimal careAttends departmental meetings and provides constructive recommendations for areas of improvementDocuments pertinent patient information in the electronic medical record as it relates to job functionCoordinate with various providers, team members and outreach team regarding any scheduling guidelinesMeets established productivity, schedule adherence, and quality standardsUses, protects, and discloses Optum patients' protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standardsPerforms additional duties as assignedWhat are the reasons to consider working for UnitedHealth Group? The hourly pay for this role will range from $16.00 to $29.00 per hour based on full-time employment.
