Job Title :- Nurse Practitioner
Location :- Commack, NY 11725
Contract :- 3+ Months with possible extension

Key Responsibilities
Evaluate and manage patients with chronic and complex health conditions
Perform patient assessments and physical examinations
Diagnose conditions and develop treatment plans
Manage medications and prescriptions
Provide ongoing care coordination and follow-up
Document patient encounters accurately and timely

Requirements:
Active Adult Nurse Practitioner license
Strong clinical assessment and diagnostic skills
Experience in chronic care or occupational health preferred
