Your Day-to-Day Life at Custom:

As a Physical Therapist at Custom Home Health, you will:

• Deliver individualized care that empowers patients to achieve their health and independence goals.
• Collaborate with RNs and interdisciplinary teams to design and implement personalized care plans.
• Educate patients and their families on therapy techniques, exercises, and treatments to promote lasting well-being.
• Coordinate and administer treatments that enhance physical function and overall health.
• Approach every task with compassion, professionalism, and a commitment to excellence.

What We're Looking For:

• Licensed Physical Therapist : Must hold a valid Physical Therapy license.
• 1+ Years of Physical Therapy Experience : Prior experience preferred, but exceptional new graduates are encouraged to apply.
• Passion for Excellence : A genuine desire to provide top-tier care to our patients and community.
• Team Player : Strong collaboration and communication skills.
