About the Role:

Responsibilities:

• Provide direct patient care, including administering medications and monitoring vital signs.
• Assist with daily living activities and personal care for hospice patients.
• Collaborate with the healthcare team to develop and implement individualized care plans.
• Educate patients and families on care options and end-of-life issues.
• Document patient progress and communicate changes to the nursing staff and physicians.
• Participate in team meetings and contribute to quality improvement initiatives.
• Ensure compliance with all regulatory and safety standards in hospice care.
• Provide emotional support to patients and families during challenging times.

Requirements:

• Current LPN license in the state of Georgia.
• Minimum of 1 year experience in hospice or palliative care preferred.
• Strong clinical assessment and communication skills.
• Ability to work independently and as part of a multidisciplinary team.
• Compassionate demeanor with a commitment to patient-centered care.
• Basic Life Support (BLS) certification required.
• Flexibility to work various shifts, including weekends and holidays.
• Reliable transportation and willingness to travel to patient locations.
