Hospice Registered Nurse (RN) Case Manager Hiawatha, IA This caring hospice organization recognizes the significance of each precious, end-of-life journey. We will do all we can to provide an empowering hospice experience to each patient we have the privilege of working with. We are seeking a talented Hospice Registered Nurse (RN) Case Manager for 40 hours per week . Job Summary The Hospice Registered Nurse (RN) Case Manager plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education and experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. Responsibilities Completes an initial, comprehensive, and ongoing assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illnesses. Provides professional nursing care by utilizing all elements of the nursing process. Assesses and evaluates the patient’s status by: Writingprovements and initiating plan of care Regularly re-evaluating patient and family/caregiver needs Participating in revising the plan of care as necessary Initiating the plan of care and making necessary revisions as the patient’s status and needs change. Using health assessment data to determine nursing diagnosis. Developing a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions, including the patient and the family in the planning process. Initiating appropriate preventive and rehabilitative nursing procedures. Administering medications and treatments as prescribed by the physician in the physician’s plan of care. Counselling the patient and family in meeting nursing and related needs. Providing health‑care instructions to the patient as appropriate per assessment and plan. Assisting the patient with activities of daily living and facilitating the patient’s efforts toward self‑sufficiency and optional comfort care. Acting as RN Case Manager when assigned by the Clinical Supervisor and assuming responsibility to coordinate patient care for the assigned caseload. Participating in on‑call duties as defined by the on‑call policy. Ensuring that arrangements for equipment and other necessary items and services are available. Supervising ancillary personnel and delegating responsibilities when required. Assuming responsibility for personal growth and development and maintaining and upgrading professional knowledge and practice skills through attendance and participation in continuing education and in‑service classes. Fulfilling the obligation of requested and/or accepted case assignments. Actively participating in quality assessment, performance‑improvement teams and activities. Communication Completing, maintaining, and submitting accurate and relevant clinical notes regarding the patient’s condition and care provided. Recording pain/symptom‑management tad changes/outcomes as appropriate. Communicating with the physician regarding the patient’s needs and reporting changes in the patient’s condition; obtaining/receiving physicians’ orders as required. Communicating with community‑health‑related persons to coordinate the care plan. Teaching the patient and family/caregiver self‑care techniques as appropriate; providing medication, diet and other instructions as ordered by the physician and recognizing and utilizing opportunities for health counseling with patients and families/caregivers. Working in concert with the interdisciplinary group. Providing and maintaining a safe environment for the patient. Assisting the patient and family/caregiver and other team members in providing continuity of care. Working in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver. Attending interdisciplinary diplomastic meetings. Qualifications A passion for hospice care and the ability to build relationships with both patients and their family members. The ability to work efficiently with little direct supervision. A valid driver’s license and an insured automobile that is in good working order. 1‑2 years of recent acute‑care experience in an institutional setting (required). 1‑2 years of recent home‑care experience (preferred). We’re excited to train RN’s that are passionate about hospice care. Graduation from an accredited school of nursing. Understanding hospice philosophy, secure with issues of death/dying, and comfortable providing specialized care to the terminally ill. Displaying empathy for the needs of the ill, injured, frail and impaired. Providing a calm demeanor when in a patient’s home. Benefits of Employment Excellent compensation. Coverage Area Will be assigned a centralized territory. Productivity Managing an average caseload of 12‑15 patients. EMR Used Homecare Homebase + nVoq voice app for seamless documentation.
