Job Summary: We are seeking a skilled and compassionate Licensed Practical Nurse to join our dedicated team at Emerald Nursing & Rehab Legacy Pointe. As an LPN, you will play a vital role in providing high-quality resident care to our residents, working collaboratively with our interdisciplinary team to achieve individualized care plans. Responsibilities: Provide high-quality resident care to residents, including administering medications, wound care, and other treatments as directed by the medical staff. Perform vital signs, monitor patient conditions, and report any changes or concerns to the nursing team. Develop and implement individualized care plans, collaborating with the interdisciplinary team to ensure resident needs are met. Maintain accurate and detailed records of resident care, treatments, and progress. Participate in quality improvement initiatives to enhance patient outcomes and satisfaction. Foster a positive and respectful environment, promoting dignity and comfort for our residents. Requirements: Current licensure as a Licensed Practical Nurse (LPN) in the state of Nebraska. High school diploma or equivalent required. Basic Life Support (BLS) certification. Ability to effectively communicate with residents, families, and healthcare team members. Strong interpersonal and teamwork skills, with a focus on providing excellent patient care.
