Roles & Responsibilities

• Evaluate and treat patients with physical impairments in our 74-bed skilled nursing facility
• Develop individualized care plans to restore function and improve mobility
• Collaborate with the interdisciplinary team to ensure comprehensive care
• Educate patients and families on therapeutic exercises and techniques
• Document patient progress accurately and timely
• Participate in care conferences and coordinate discharge planning

Qualifications

• Current New Jersey Physical Therapist license
• DPT or Master's degree in Physical Therapy (new graduates welcome)
• Experience in skilled nursing or long-term care is preferred
• Strong clinical and communication skills
• A passion for improving quality of life for elderly patients

Knowledge & Skills

• Expertise in rehabilitation techniques and therapeutic exercise
• Ability to perform patient assessments and create treatment plans
• Knowledge of Medicare and insurance documentation requirements
• Excellent interpersonal and teamwork skills
• Proficiency with electronic health records
