Registered Nurse (RN) MDS Coordinator/NAC
At Briarwood Nursing and Rehab in Flint, Michigan, United States , we are seeking a skilled and compassionate Registered Nurse (RN) MDS Coordinator/NAC to join our team.
About the Role
We are looking for a dedicated and experienced RN to coordinate the care of our residents by gathering and analyzing data to develop and implement effective care plans. As an MDS Coordinator/NAC, you will be responsible for ensuring that our residents receive the highest level of care, while also maintaining compliance with regulatory requirements. In this part-time role, you will have the opportunity to work closely with our interdisciplinary team to make a meaningful difference in the lives of our residents.
Responsibilities
Coordinate the care of residents by gathering and analyzing data to develop and implement effective care plans
Ensure that residents receive the highest level of care by working closely with the interdisciplinary team
Maintain accurate and detailed records of resident care and services
Conduct regular assessments to identify areas for improvement and implement changes as needed
Collaborate with other departments to ensure seamless communication and coordination of care
Stay up-to-date with regulatory requirements and maintain compliance with all relevant standards
Participate in educational and training programs to stay current with industry best practices
Requirements
Current RN license in the state of Michigan
Experience in long-term care or related field
Strong assessment and problem-solving skills
Excellent communication and interpersonal skills
Ability to work independently and as part of a team
