Registered Nurse (RN) - Hospice

Hospice Admission Visit RN

Schedule:

Days: Sunday - Thursday, 10am - 7pm or 11am - 8pm (Flexible based on patient's availability). OR Days: Tuesday - Saturday, 10am - 7pm or 11am - 8pm (Flexible based on patient's availability). One weekend per month.

Assignment Details:

Holiday coverage required.

Requirements:

• One year of Hospice experience.
• BLS/BCLS certification.

Responsibilities:

• Admit patients to service and assess the patient's and family's needs.
• Develop individualized plans of care based on assessments.
• Document observations and interventions related to the patient's environment.
• Coordinate care with a multidisciplinary team.
• Participate in quality assessment performance improvement programs.
• Conduct follow-up visits as needed.
