Bertrand Chaffee Hospital in Springville, New York, is seeking an experienced and dedicated Pharmacist to join our team!


Job Summary:

Schedule is 7am-3pm Fridays and Saturdays, with additional hours available as needed. You will monitor our medication delivery system including, med orders, transcriptions, IV admixture, extemporaneous products, dosages, routes of administration, and frequency and duration of therapy for the hospital and for the nursing home. You will play a vital role in providing high-quality patient care and support to our patients, families, and communities.

Responsibilities:

• Monitor patient profiles for drug efficacy and interactions.
• Verify compatibility of ordered drugs and compounds.
• Identify patients' home medications; inform staff and clarify, interpret, and enter admission orders.
• Check physician orders, computer entries, and medications for accuracy.
• Provide plain-language medication education to patients when requested.
• Complete medication reconciliation for home medications.
• Maintain controlled substance security and accountability per policy.
• Support formulary compliance and consult physicians on availability and alternatives.
• Train new pharmacy staff and oversee pharmacists' and assistants' activities.
• Educate pharmacy students and graduates.
• Maintain accurate drug inventory control.

Requirements:

• License to practice as a Pharmacist in New York State.
• Bachelor's Degree/PharmD in Pharmacy from an ACPE accredited institution.
• Experience within the hospital environment strongly preferred.
