Job Title: Travel RN Case Manager Job ID: 3389631 Facility: Providence Saint Mary Medical Center – Apple Valley Location: Apple Valley, CA Start Date: August 17, 2026 Duration: 13 Weeks Shift: Days | 5x8s | 8:00 AM – 4:30 PM Weekly Gross Pay: $2,600 Requirements Minimum 2 years of Acute Care Case Management and Discharge Planning experience ( Required ) Seasoned travel RN required ( First-time travelers will not be considered ) Active California RN License (Pending license accepted, must be active by start date) Epic experience required Recent Acute Care Hospital experience required Key Responsibilities Patient Assessment & Care Coordination Disease Management Needs Assessment & DME Coordination Prior Authorizations Development of Individualized Care Plans Family Meetings & Multidisciplinary Collaboration Arrange SNF Placements, DME, and Ambulance Transportation Ensure CMS, Joint Commission, and Department of Health compliance Preferred Experience Emergency Department Medical-Surgical ICU Surgical Services Experience working with HMOs, IPAs, and managed care Knowledge of homelessness resources, substance abuse treatment, mental health services, foster/adoption processes, advance directives, POLST, and protective services Facility Highlights Epic charting Weekend rotation required (approximately every other weekend) No floating required Business attire or scrubs permitted #J-18808-Ljbffr KTI Health
