Agency Flex Social Worker

Location: Agency Wide

Schedule: Monday-Friday 8am-5pm occasional on-call rotation and overnight travel may be required

The responsibilities outlined will require travel to any BCN regional office which may include overnight accommodations.

As a BCN Hospice Team member you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home. While this often is a house or apartment we also provide care at nursing homes assisted living facilities and hospitals. If a patients care needs are too complicated for home care or require a higher level of attention from nurses and other experts we can provide care at one of our Hospice Care Centers.

About the Role:

A team member in the Agency Flex Social Worker position provides direct casework and counseling services to the patients and families within Bluegrass Care Navigators Hospice Care Program. The Agency Flex Social Worker will provide an assessment of psychosocial problems define treatment goals and work to alleviate such problems. The services provided by the Agency Flex Social Worker may require visits in a home or nursing home facility anywhere within in the Bluegrass Care Navigators service regions. Therefore travel may be required including overnight accommodations at times.

The Agency Flex Social Worker serves as part of the interdisciplinary team and shares responsibility for other aspects of patient care except where limited by licensure. This position also plans and coordinates patient care with the primary nurse under the direction of the Regional Counseling Supervisor.
