Float Case Manager

Humana is seeking a Float Case Manager to support members across diverse populations by providing flexible, telephonic, and virtual case management. In this role, you will rotate among teams and lines of business, performing comprehensive assessments, creating person-centered care plans, and coordinating services that improve access, quality, and outcomes. You will partner with clinicians, community resources, and families to address medical, behavioral, and social needs.
