DEPARTMENT SUMMARY: The Pharmacy Services Division at St. Charles Health System provides comprehensive acute, infusion, ambulatory, retail, and specialty pharmaceutical care. The division ensures medication safety, quality, regulatory compliance, and financial stewardship across the continuum of care.

POSITION OVERVIEW:

Fulfills the duties of the Pharmacist-in-Charge (PIC) as outlined by the Oregon Board of Pharmacy (OAR 855-019-0300). Provides pharmaceutical care as a complete package across the hospital medication process in order to increase the effectiveness and quality of patient care. This position does not directly manage any other caregivers.

LOCATION: Pharmacists may be asked to work at multiple St. Charles Health System worksites at the request of leadership to fulfill short term, immediate staffing needs.

ESSENTIAL FUNCTIONS AND DUTIES:

In collaboration with the CPO and pharmacy leadership, assists with design, deployment, and sustainment of Oregon Board of Pharmacy standards.

Participates in and represents pharmacy leadership at huddles, committees, and work groups as assigned.

Maintains alignment with pharmacy system goals and initiatives.

Coordinates and oversees students and preceptor duties for the pharmacy.

Provides specialized pharmaceutical services to improve drug usage and therapeutic outcomes including, but not limited to, advising physicians on issues concerning drug therapy, the inherent toxicity of drugs and their side effects, as well as assisting in the prescription of appropriate doses.

Coordinates the activities of assigned pharmacy technicians engaged in the preparation, labeling and delivery of medications and other pharmaceutical supplies in accordance with physicians' prescriptions; ensures compliance with federal, state and local laws and regulations pertaining to the dispensing of drugs and controlled drugs and the maintenance of required records.

Mentors and trains others in training including practical experiences.

Maintains liaison relationships with medical and nursing staff; provides timely information pertaining to pharmaceutical supplies, drug usage and compatibility, state and federal regulations regarding drug controls, and Joint Commission Standards.

Conducts and evaluates medication histories, assesses compliance and suggests modifications so as to achieve desired outcomes; instructs patients in the proper use of prescribed drugs and makes Patient Care Rounds with physicians to evaluate patient progress.

Individualizes medication regimens using sound principles, accounting for pharmacodynamic and pharmacokinetic variations in drug absorption, distribution, metabolism and elimination with responsibility for establishing and continually improving the delivery of Pharmaceutical Care to patients within areas of direct responsibility and assisting others in the department with the same.

Extracts pertinent information from patient/other sources for the patient's medical management plan.

Maintains flexibility and readiness to work in multiple worksites either at the request of, or in mutual agreement with, leadership.

Operates pharmacy computer system(s) and other equipment.

EDUCATION:

Required: Bachelor of Science degree in Pharmacy.

Preferred: PharmD., Completion of an ASHP accredited PGY-1 residency training


LICENSURE/CERTIFICATION/REGISTRATION:

Required : Current registration by the Oregon Board of Pharmacy. AHA Basic Life Support for Healthcare Provider certification.


Preferred : Any certification through the Board of Pharmacy Specialties (BPS).

EXPERIENCE:

Required: Minimum one (1) year of pharmacy practice and completion of OBOP approved PIC training course within first year of appointment.


Preferred: Previous PIC experience.


PERSONAL PROTECTIVE EQUIPMENT:

Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely.

PHYSICAL REQUIREMENTS:

Continually (75% or more): Standing and walking, carrying 1-10 pounds, operation of a motor vehicle for caregivers that are couriers.

Frequently (50%): Lifting 1-10 pounds, pushing or pulling 1-25 pounds, grasping/squeezing.

Occasionally (25%): Sitting, bending, climbing stairs, lifting and carrying 11-25 pounds,

Use of clear and audible speaking voice and ability to hear normal speech level.

Rarely (10%): Keyboard operation, stooping/kneeling/crouching, reaching overhead, pushing or pulling 25-50 pounds.

Never (0%): Climbing ladder/stepstool, lifting or carrying 25-50 pounds, whispered speech level, operation of a motor vehicle.
