Responsibilities Provides and ensures nursing care to residents on the nursing unit within the framework of the medical and nursing care plan, policies of the Department, and under the supervision of a professional nurse. Monitors residents to identify response to treatments, medications, and changes in health status; provides treatments according to the medical and nursing care plan and department policies. Prepares and dispenses medications to residents according to the medical plan of care and department policies. Directly assists with activities of daily living, including direct care, observation of status and changes, and supervision of non-professional staff. Documents resident data in progress notes; assists with weekly, monthly, and annual documentation of resident status and care. Communicates changes and incidents to the immediate supervisor; completes required documentation (e.g., incident/accident reports, infection control reports, supervisory notes, shift reports). Maintains policies and procedures relevant to medications, including management of controlled substances. Communicates pertinent information and changes impacting resident care to supervisor, nursing staff, and other departments. Responds to resident and family concerns; delivers care aligned with assessments, resident requests, and plan of care goals. Works with other professionals and disciplines to ensure timely and coordinated care. Participates in the Department’s Quality Assurance Program and serves on committees related to quality of care. Participates in the orientation of new employees and the clinical in-service of non-professional staff. Qualifications Required Qualifications: NYS Licensed Practical Nurse (LPN) License. Preferred Qualifications: One year of healthcare experience with adult or aging populations in a clinical setting. One year of long-term care experience. Experience using EMR.
