Resident Care CoordinatorThe Resident Care Coordinator delivers quality assisted living services focused on maximizing resident's individual capabilities in accordance with all laws, regulations and Century Park standards. Reports to Resident Care Director (or Assistant Resident Care Director).QualificationsMust possess a high school diploma or equivalentCertified Nurses Aide (CNA) certification, or Med Tech or QMAP certification preferredMust have minimum of one (1) year experience in senior servicesMust have CPR certification upon hire or obtain during Orientation. Certification must remain current during employment.Primary Job ResponsibilitiesProvides assistance with Resident Care staff scheduling, including coverage for planned and unplanned absencesProvides assistance with scheduling meetings with families, healthcare providers, home health, etc.Assists new hires with on the job training and onboarding in the Resident Care departmentPerforms duties as care aide, QMAP, Med Tech or CNA as needed and as licensure allowsAssists with receiving physician orders, transcribing to medication sheets and charts appropriatelyProvides assistance with communication with residents, families, physicians, hospice, and vendorsProvides assistance with entering resident information in the Electronic Health Record (HER) systemAssists with coordinating wellness clinics, flu clinics, ancillary appointments (i.e. podiatry), and transportation needsProvides assistance with lab orders, preparing new resident charts, and proper disposition of residents charts who have moved away or expiredMay assist with medication management refills, delivery to med carts, expired/discontinued meds (as State licensure allows)Assists resident with care needs when necessary (as State licensure allows)Additional RequirementsMust enjoy serving seniorsMust have patience, tact, cheerful disposition, and enthusiasm as well as a willingness to handle difficult residentsMust possess the ability to make independent decisions when circumstances warrant such actionMust perform efficiently and effectively with little to no supervisionMaintains professional working relationships with all associates, vendors, etc.Maintains confidentiality of all proprietary and/or confidential informationDisplays integrity and professionalism by adhering to Century Park's Code of Ethics and completes all required compliance trainingWho We AreAt Century Park Associates, we are dedicated to creating an enriching lifestyle for our residents and a rewarding work environment for our valued associates.
