Registered Nurse Hospice

$1996.48/week -

Job Summary:

Provide comprehensive hospice nursing care to patients in a home setting, coordinating activities and ensuring quality adherence to physicians' orders. Will evaluate patient conditions, supervise home health aides and licensed practical nurses, and document all care activities accurately.

Responsibilities:

• 
  Coordinate and deliver patient care according to physicians' orders and agency policies.
  
  
• 
  Supervise and evaluate home health aides and licensed practical nurses ensuring high-quality treatment.
  
  
• 
  Perform start of care, wound care, follow-up visits, and other specialized nursing tasks.
  
  
• 
  Document patient progress, interventions, and treatment plans accurately within the required time frame.
  
  
• 
  Travel up to 75 miles from the assigned clinic to deliver patient care.
  
  
• 
  Participate in periodic office meetings and maintain proficiency in EMR systems.
  
  

Qualifications:

• 
  Hold an active and valid nursing license in the state of assignment.
  
  
• 
  Minimum of 1 year of recent adult home health or hospice experience.
  
  
• 
  Proficient in using HomeCare HomeBase and OASIS documentation systems.
  
  
• 
  Demonstrate excellent clinical assessment, communication, and organizational skills.
  
  
• 
  Must have reliable transportation, a valid drivers license, and current liability insurance.
  
  
• 
  Ability to travel within designated territory and work flexible schedules, including weekends and on-call rotations.
