Evening and NightResponsibilities for this position include, but are not limited to:Provide direct nursing care to residents in accordance with physician orders and facility policies.Administer medications, treatments, and nursing interventions safely and accurately.Monitor, assess, and document resident conditions, vital signs, and responses to treatment.Assist residents with activities of daily living (ADLs) as needed.Collaborate with physicians, RNs, CNAs, and other healthcare professionals to develop and implement individualized care plans.Maintain accurate and timely documentation in the electronic medical record system.Observe and report changes in resident conditions to the appropriate healthcare team members.Ensure compliance with infection control, patient safety, and regulatory standards.Provide emotional support and education to residents and families.Participate in shift reports, interdisciplinary meetings, and quality improvement initiatives.To be considered for this position, you should have:Active New York State LPN License (Required)Current BLS Certification (Preferred/Required)Minimum 1–2 years of Long-Term Care or Skilled Nursing experience preferredStrong medication administration and patient assessment skillsExcellent communication and teamwork abilitiesAbility to work independently in a fast-paced environmentKnowledge of geriatric care and chronic disease management preferredTryfacta is an Equal Opportunity/Affirmative Action Employer.
