Role Description Empress Healthcare Inc. is seeking a licensed Physical Therapist (PT) to provide skilled home health physical therapy services as an independent contractor (1099) . This is a flexible, per-visit opportunity for a clinician who values professional autonomy, high-quality patient care, and interdisciplinary collaboration. Physical Therapists may accept or decline case assignments and coordinate their own visit schedules based on patient availability, clinical needs, and required case timelines. The Physical Therapist will evaluate patients in their homes, develop and update individualized physical therapy treatment plans within their scope of practice, provide skilled therapeutic interventions, monitor patient progress, and collaborate with physicians and the interdisciplinary care team. Key Responsibilities Perform comprehensive initial physical therapy evaluations and periodic reevaluations. Develop and update individualized physical therapy treatment plans consistent with physician orders and applicable standards of practice. Provide skilled therapeutic interventions, including: Therapeutic exercise Transfer training Therapeutic activities Balance and mobility training Fall-prevention and home-safety education Assistive-device training Educate patients and caregivers regarding home exercise programs, mobility techniques, safety strategies, and functional independence. Monitor patient progress and identify changes in functional or clinical status. Communicate patient progress, significant findings, and recommended changes to physicians and the interdisciplinary care team. Perform required functional reassessments in accordance with Medicare, agency, and professional standards. Supervise Physical Therapist Assistants (PTAs), when assigned, in accordance with applicable California requirements and agency policies. Complete accurate, timely clinical documentation and visit notes. Collaborate with RN case managers, physicians, and other members of the interdisciplinary team regarding care coordination and discharge planning. Promptly report significant changes in condition, incidents, or patient-safety concerns to the appropriate clinical supervisor. Qualifications Required Doctor of Physical Therapy (DPT) or equivalent degree from a CAPTE-accredited physical therapy program. Active and unrestricted California Physical Therapist license in good standing. Current CPR/BLS certification . Valid California driver's license and current automobile insurance. Ability to travel independently to patients' homes within the accepted service area. Strong clinical judgment and ability to independently manage a mobile home-health caseload. Excellent interpersonal, communication, and documentation skills. Professionalism, cultural sensitivity, and commitment to patient-centered and nondiscriminatory care. Ability to maintain HIPAA/PHI confidentiality and comply with applicable Medicare, state, and agency requirements. Current professional liability/malpractice insurance as required under the Independent Contractor Agreement. Ability to meet agency health-screening, orientation, competency, and credentialing requirements prior to providing patient care. Preferred Previous home health physical therapy experience . Previous outpatient rehabilitation experience. Familiarity with Medicare home health documentation and Conditions of Participation . Experience coordinating care with physicians, nurses, PTAs, and interdisciplinary home health teams. Physical & Travel Requirements The Physical Therapist must be able to perform the physical functions ordinarily associated with home health physical therapy, including assisting with patient mobility and transfers, bending, kneeling, reaching, standing, walking, and navigating a variety of residential environments. The position also requires independent travel between patient homes and the ability to work safely in varied home environments while following standard infection-control and safety precautions. This position is structured as a 1099 independent contractor engagement and is compensated on a per-visit/per-case basis pursuant to the Independent Contractor Agreement. Contractors May accept or decline available patient assignments. Coordinate their own patient visit schedules within required clinical timelines and patient availability. Are not assigned fixed shifts or a fixed weekly schedule. Provide their own vehicle and required professional treatment equipment. Are responsible for applicable self-employment taxes and other obligations associated with independent contractor status. Specific compensation rates, invoicing procedures, payment schedules, insurance requirements, and other engagement terms are provided through the Independent Contractor Agreement.
