Registered Nurse Case Manager — Remote/Field

Pay Rate: $45–$46/hour maximum
Schedule: Monday–Friday, 8:00 AM–5:00 PM
Work Arrangement: Fully Remote / Telephonic & Field Work
Location Requirement: Candidates must reside in one of the following Virginia cities:

• Falls Church
• Annandale
• Springfield
• Arlington
• Alexandria

Position Summary

The RN Case Manager utilizes a collaborative process of assessment, planning, facilitation, and advocacy to coordinate options and services that meet members' benefit plans and healthcare needs. The Case Manager works with members, providers, and available internal and external resources to promote optimal, cost-effective outcomes.

This position involves telephonic and field-based case management and requires an active, unrestricted Registered Nurse license in Virginia or a valid compact-state RN license.

Key Responsibilities

• Conduct comprehensive assessments of referred members' healthcare needs and eligibility using clinical tools and available data.
• Evaluate member benefit plans and available internal and external programs and services to determine appropriate case resolution.
• Develop and implement individualized case management approaches and plans.
• Apply applicable clinical criteria, guidelines, policies, procedures, and regulatory standards when assessing member needs and benefits.
• Coordinate services and resources to support appropriate healthcare outcomes.
• Utilize case management and quality management processes in accordance with regulatory, accreditation, and company requirements.
• Communicate effectively with members, providers, healthcare professionals, and other stakeholders.
• Navigate multiple computer systems while documenting case management activities accurately and efficiently.
• Manage multiple priorities in a fast-paced and changing environment.
• Participate in field activities as required by the role.

Required Qualifications

• Active, unrestricted RN license in Virginia or a valid compact-state RN license.
• RN license must be in good standing with no history that would restrict practice .
• Minimum 3 years of clinical practice experience , such as:
  • Hospital setting
  • Home health
  • Ambulatory care
  • Other comparable clinical environments
• Healthcare and/or managed care industry experience.
• Proficiency with computers, including navigating multiple systems and keyboarding.
• Strong verbal and written communication skills.
• Ability to multitask, prioritize, and adapt effectively to a fast-paced environment.
• Verifiable High School Diploma or GED.

Preferred Qualifications

• Previous Case Management experience.
• CCM (Certified Case Manager) certification preferred.
• Experience with managed care or health plan operations.

Work Environment & Requirements

• Fully remote — no onsite reporting required.
• Role includes telephonic and field work .
• Candidates must have a quiet, dedicated workspace free from distractions .
• Work involves extended periods of sitting, talking on the telephone, listening, and working at a computer.
• Ability to closely inspect handwritten and computer-generated documents and monitor screens.
• Must be able to maintain productivity and quality expectations in a remote work environment.
