Director of Case Management & House Supervisors

The OpportunityMinden Medical Center is seeking a Case Management Director and House Supervisor to guide patient-centered care coordination and provide operational oversight across hospital units on days. You’ll spend time rounding to stay connected to patient and family needs, fielding real-time questions from staff, and keeping communication clear and respectful. Midday often includes reviewing documentation and key metrics, followed by coordinating with community resources and internal departments to keep plans moving. As the day wraps, you’ll ensure smooth handoffs, confirm follow-ups are in place, and leave a clear status update for the next leader on duty.
