Home Health Physical Therapist Part Time PRN

Position Overview We are seeking a Physical Therapist (PT) to assess, teach, and deliver therapy interventions based on physician-directed home health plans. The role involves collaborating with an interdisciplinary team, providing updates to optimize patient well-being, and performing initial and ongoing assessments of mobility, strength, and function. Evaluate patient mobility, coordination, and function to set goals and outcomes. Develop and document plans of care, including progress and discharge strategies. Deliver physical therapy services in accordance with physician orders and maintain accurate documentation. Participate in discharge planning, educate patients and caregivers on home exercises, and coordinate with healthcare team members. Supervise relevant personnel (PTA, HHA) and complete assessments within scope of practice. Qualifications And Skills Degree in Physical Therapy from an accredited program. Current, unrestricted state license as a Physical Therapist. Proficiency in clinical assessments, documentation, and compliance with policies.
