We are seeking a dedicated Preadmission Screening and Resident Review (PASRR) to provide assessment and care coordination for seniors and adults in nursing facilities. This role focuses on conducting PASRR evaluations for individuals with potential mental health or intellectual/developmental disabilities. About the Role: The PASRR Case Manager plays a critical role in ensuring that individuals with serious mental illness or intellectual disabilities receive appropriate assessments and services in compliance with the Preadmission Screening and Resident Review (PASRR) federal mandate. This position involves conducting thorough evaluations to determine the level of care needed and coordinating with healthcare providers, facilities, and families to develop and implement individualized care plans. The Case Manager acts as an advocate for the rights and needs of residents, ensuring they are placed in the most suitable care settings and receive necessary supports. This role requires meticulous documentation and reporting to maintain compliance with state and federal regulations. Ultimately, the PASRR Case Manager contributes to improving the quality of life for vulnerable populations by facilitating access to appropriate long-term care services and supports. Minimum Qualifications: Bachelor’s degree in social work, psychology, nursing, or a related human services field. Must meet Qualified Mental Health Professional (QMHP) requirement. Three years of full-time work experience in human services case management. Must have a valid Texas driver's license, a good driving record in the past 3 years, and valid auto insurance coverage. Strong organizational skills with the ability to manage multiple cases simultaneously. Excellent communication skills, both written and verbal. Preferred Qualifications: Master’s degree in social work, counseling, or a related field. Knowledge of PASRR regulations and federal/state long-term care policies. Experience working specifically with individuals with intellectual disabilities or serious mental illness. Familiarity with electronic health records (EHR) and case management software. Training in trauma-informed care or person-centered planning. Responsibilities: Conduct comprehensive PASRR Level I and Level II screenings and assessments for individuals seeking admission to nursing facilities. Collaborate with healthcare providers, social workers, and family members to develop individualized service plans that address the unique needs of each resident. Monitor and review ongoing care to ensure compliance with PASRR requirements and adjust care plans as necessary. Maintain accurate and detailed records of assessments, care plans, and communications in accordance with regulatory standards. Provide education and guidance to facility staff and families regarding PASRR processes, resident rights, and available community resources. Coordinate with state agencies and other stakeholders to facilitate timely and effective service delivery. Advocate for residents to ensure their needs are met and their rights are protected throughout the care continuum. Skills: The PASRR Case Manager utilizes strong assessment and analytical skills daily to evaluate individuals’ needs accurately and develop appropriate care plans. Effective communication skills are essential for collaborating with multidisciplinary teams, advocating for residents, and educating families and facility staff. Organizational and time management skills enable the Case Manager to handle multiple cases efficiently while maintaining compliance with regulatory requirements. Proficiency with documentation and data management tools ensures accurate record-keeping and reporting.
