Physical Therapist

Shift: 8 hours

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 40

Requirements:

• Current State License as a Physical Therapist
• Proof of current CPR certification
• Current driver's license with reliable transportation and insurance
• 1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting
• Previous home care experience - Preferred
• Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program

Roles & Responsibilities:

• Perform clinical assessments to identify patient needs and appropriateness of care.
• Document, prepare, and review Plans of Care, progress notes, and other clinical documentation on an ongoing basis. Revise Plans of Care based on evaluation and ongoing assessment data.
• Plan and provide appropriate treatment for neuromuscular, neurological, and orthopedic disorders based on medical referrals and therapy assessments.
• Assist and instruct patients and families in transfer techniques and the safe use of adaptive equipment.
• Provide patient and family education regarding therapeutic procedures based on the patient's age, condition, and learning readiness.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.

Compliance / Notes:

• Applies infection control and universal precautions in patient care activities.
• Accepts clinical assignments consistent with education and competence.
• Must meet mandatory continuing education requirements.
