Travel Physical Therapist Opportunity In Albemarle, NC

Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active North Carolina state PT license or ability to obtain state licensure prior to the start of assignment.
• Minimum of 1 year of Home Health physical therapy experience required. First-time travelers accepted if experienced in home health (preferred).
• Homecare Homebase EMR experience required.
• Experience with geriatric care, chronic disease management, and maintenance OASIS-E experience required.
• Competence in gait training, balance training, non-pharmaceutical pain management, and developing efficient plans of care to maximize patient function.
• Ability to recognize signs and symptoms to prevent readmissions and communicate effectively with the clinical team.
• Basic Life Support (BLS) certification required from AHA or ARC.
• Valid driver's license and proof of insurance required. Must be willing to drive extended daily mileage (average 6080 miles per day).
• Preferred: Experience with EPIC Rover (view only), Focura, and Samsung Note devices used for documentation and media upload.

Assignment Details

• Facility Type: Home Health
• Shift Name: Days
• Shift Hours: 8:30 am 5:00 pm, Monday Friday
• Standard Hours Per Week: 40
• Schedule Cycle: every 13 weeks
• Orientation: New employee orientation plus 5 days of unit orientation
• Patient Population: All adults with diagnoses including chronic disease, orthopedics (total hips/knees), falls, CHF, COPD, occasional stroke
• Typical Caseload: Approximately 6 visits per day
• Travel/Driving Expectations: No maximum mileage; average 6080 miles per day. Mileage reimbursed weekly.
• Floating: Clinician will travel throughout the regional area (Albemarle and surrounding towns). No specific facility names required.
• Call/Weekends: No on-call. No weekend rotation. No major holiday coverage.
