The Case Manager participates in the development of the integrated comprehensive assessment and Integrated Service Plan (ISP), implements services to members in accordance with their plan, provides ongoing care and reviews progress, coordinates with all involved healthcare, behavioral health and wellness providers, and maintains current documentation on members’ progress in managing chronic medical and psychiatric conditions and assists the individual in achieving their recovery goals. Key Responsibilities Work collaboratively with the integrated care team to engage, educate, communicate, and coordinate care with service participants, their families, behavioral health, medical, and dental providers, community resources and others in ensuring that all services prescribed in the Integrated Service Plan are implemented. Develop, maintain and manage the Integrated Service Plan (ISP) within the required time‑frame. Participate in the development of the integrated comprehensive assessment and Integrated Service Plan (ISP). Implement services to members in accordance with their plan and provide ongoing care and review progress. Coordinate with all involved healthcare, behavioral health, and wellness providers and maintain current documentation on members’ progress in managing chronic medical and psychiatric conditions and assist the individual in achieving their recovery goals. Ensure compliance & adequate services; additional job duties may be required to meet the needs of the program and/or department. Qualifications High school diploma required, and minimum four (4) years of experience in behavioral health and SMI, or Bachelor’s degree in a related field with one (1) year of related experience. AZ valid driver’s license, vehicle with state minimum insurance coverage and able to meet Copa Health company driving requirements and safety‑sensitive requirements. Safety‑sensitive tasks are any duties that the employer in good faith believes could affect the safety or health of the employee performing the task or others. Bilingual (English/Spanish) preferred. Compensation Starting pay: $18.50–$22.50. Copa Health is an Equal Opportunity Employer.
