Requirements:

• Active RN License
• Minimum 2 years of recent Palliative Care and/or Hospice experience
• Acute Care experience required
• BLS Certification required
• Strong clinical judgment, communication, and organizational skills

Job Responsibilities:

• Manage inpatient palliative care caseload
• Conduct patient assessments and chart reviews
• Lead goals-of-care and end-of-life discussions
• Educate patients and families on disease processes, POLSTs, and advance directives
• Participate in interdisciplinary rounds and family meetings
• Coordinate care plans with physicians and care teams
