Seven Healthcare are seeking a skilled Licensed Physical Therapist for a contract assignment in Washington County, Maryland. This role involves providing home health physical therapy services, focusing on oasis and home therapy assessments, visits, and treatments. The position offers a consistent weekday schedule with no holiday, weekend, or on-call requirements.

Key Responsibilities of the Licensed Physical Therapist
- Conduct physical therapy evaluation and treatment based on physician orders/referrals
- Complete comprehensive evaluations using appropriate tools and procedures
- Review patient records for diagnosis, medical history, and treatment precautions
- Establish measurable goals and develop individualized treatment plans
- Re-assess and modify treatment plans as necessary for patient progress
- Communicate patient needs and progress with the treatment team and family members
- Consult with medical providers regarding patient evaluation and treatment
- Assist in securing and implementing adaptive equipment to enhance rehabilitation
- Maintain documentation according to regulations and company policies
- Supervise Physical Therapist Assistants and Aides in patient care activities
- Participate in staff development and maintain clinical practice skills
Requirements of the Licensed Physical Therapist
- Valid Physical Therapy license in the state of Maryland
- Experience with Home Care Home Base is preferred
- Prior experience in home health physical therapy is a plus
