Requirements:

• Active unrestricted license in any State or U.S. territory

• Completion of 48 months of OB/GYN residency training

• BC/BE

• BLS/ACLS

• Current, unrestricted DEA license with schedules II through V.

• Minimum of 6 months of recent OB/GYN work experience

• No history of adverse medical board action, loss of licensure, or suspended licensure
