ABOUT HEARTS WITH A MISSION:
Hearts With A Mission serves at-risk youth and families, by providing shelter, educational support, counseling, family reunification, transition planning, and elder companion care all with a faith-based approach.

Monday - Friday
10:00AM - 6:00PM
Schedule subject to change based on agency needs
POSITION SUMMARY
The Transitional Living Program (TLP) Skills Trainer provides skill building and behavioral services for clients in accordance to the established program guidelines and service plan. Skills Trainers will ensure that the young adult clients (ages 18-21) receive the full range of care and treatment. The TLP Skills Trainer will also plan and facilitate groups, transport clients to support their transitional planning and needs, provide skill building coaching and activities, and complete necessary program documentation. Skills trainers assist in the care and treatment of young adults by providing a consistent presence, skills training and emotional support in a safe and secure manner. All work is performed in compliance with organizational guidelines and policies, as well as all applicable laws.
SUPERVISION RECEIVED AND EXERCISED
The TLP Skills Trainer reports directly to the Transitional Living Program (TLP) Director. This is not a supervisory position. This position has an introductory period of three (3) months.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
The essential functions listed below are not intended to reflect all duties that may be assigned to this position. The organization may augment duties and/or essential functions at its discretion.
- Deliver and coordinate individual and group independent living skills training for young adult clients.
- Establish rapport by demonstrating and maintaining clear, consistent, and appropriate boundaries.
- Demonstrate alignment with the philosophy of Collaborative Problem Solving (CPS) by utilizing the approaches in interactions with youth and young adults, families, staff, and community partners.
- Actively supervise and interact with young adults both in the community and within the established milieu to maintain safety and provide services.
- Demonstrate knowledge of youth and young adults’ development and group dynamics by adjusting behavioral approaches to developmental level and group interactions.
- Provide transportation of youth and young adults, or assistance of transportation for appointments and activities.
- Provide individual and group skills training and coaching within skill areas including (but not limited to):
- Independent living/self-sufficiency, wellness, education, coping skills, navigating the medical community and community living.
- Cooking, housekeeping, money management, employment/pre-employment, accessing medical care, managing transportation needs, recreational activities, etc.
- Problem solving, de-escalation techniques, stress management, coping skills, social skills, etc.
- Prepare and present materials for groups and individual sessions.
- Complete progress notes and appropriate documentation.
- Meet with TLP team as needed to review cases.
- Attend a variety of meetings, trainings, seminars, staff meetings, etc. as requested. Some will require overnight and out of town travel.
- Develop needs and service plans.
- Act as a role model to clients; actively incorporate and demonstrate personal and professional empowerment principles and integrity.
- Safely de-escalate youth and young adults, including in crisis situations, by effectively using verbal intervention skills.
- Concisely and thoroughly document daily information. Provide timely and accurate documentation to meet billing requirements.
- Must adhere to Oregon Administrative Rules (OAR) and Standards of Care.
- Abide by ethical codes, mission, values, and professional standards, including confidentiality.
- Actively utilize, reference, and follow agency policies, procedures, and guidelines.
- Present a positive attitude, professional demeanor and demonstrated respect with youth and young adults, families, staff, visitors, and community partners.
- Perform all work tasks in a proper and safe manner per established policies, procedures and guidelines to prevent unnecessary injury, time loss and agency expense.
- Comply with all federal, state, and agency health and safety reporting requirements.
- Represent HWAM in public settings professionally and respectfully; may include public speaking.
- Attend trainings and meetings as assigned. May be appointed to committees.
- Performs other duties as assigned.

- Ability to pass a pre-employment DHS Child Welfare background check.
- Ability to maintain an acceptable driving record, reliable transportation, and proof of valid auto insurance.
- Demonstrate dependability, reliability, and accountability to include punctuality and attendance.
WORK ENVIRONMENT
Work is performed in varied residential and community environments based on assigned caseload. The conditions of a client’s home and environment may vary. This position is frequently exposed to loud noises, outside weather conditions such as heat, cold, and humidity. Occasional exposure to chemical solutions and Bloodborne Pathogens (body fluids) may occur. Risk of verbal and physical threats and harm from clients is possible. Travel is expected for required trainings and providing transportation to clients. ILP Skills Trainers will use their own vehicle for transportation of young adult clients and mileage reimbursement is available for this position. PHYSICAL AND MENTAL JOB REQUIREMENTS
To perform the job successfully, an individual must be able to perform each of the essential job functions satisfactorily. This position involves periods of prolonged sitting and standing and use of computer equipment. This position requires the physical and mental capabilities to read information printed on paper and displayed on computer monitors; hear, speak, and communicate verbally using the English language; cognitive thinking and mathematical calculation capabilities; manual dexterity to manipulate papers, files, financial records, keyboard, 10-key, mouse, and telephone. This position requires walking, crouching, bending, stooping, twisting, turning, balancing, climbing steps/stairs, regular limb extension, pinching, grasping, moving, lifting, and carrying objects of 10 or more pounds frequently. Occasional driving required locally and long distance. Specific vision abilities are required to include close vision, distance vision, and depth perception and operating a motor vehicle. MINIMUM EXPERIENCE AND QUALIFICATIONS
- One (1) year of direct experience working with at-risk clients.
- Valid Oregon driver’s license at time of hire.
DESIRABLE EXPERIENCE AND QUALIFICATIONS
- Bachelor’s Degree in Education, Social Work, Psychology, or a related field OR an equivalent combination of education and experience (i.e. some training and 3 years’ experience).
- Three (3) years’ experience in a similar position such as Skills Trainer or Case Manager.
- Bilingual in English/Spanish.
