Key Responsibilities

Evaluate and treat patients in a skilled nursing facility setting to restore function and improve quality of life
Develop and implement individualized physical therapy treatment plans based on patient needs and goals
Monitor and document patient progress, adjusting plans of care as clinically appropriate
Collaborate with interdisciplinary care team members to ensure coordinated patient outcomes
Educate patients and caregivers on therapeutic exercises and safe functional mobility

Required Qualifications

Active Physical Therapist (PT) license in the state of Tennessee
Graduate of an accredited Physical Therapy program
Previous experience in a skilled nursing or long-term care environment
Ability to work day shift hours

Preferred Experience / Skills

Prior travel or contract therapy experience
Familiarity with MDS documentation and Medicare Part A/B billing in SNF settings
Experience with restorative and rehabilitative therapy programs
