About the Role:

Rehab at Scottsdale Village Square is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. Responsibilities:

• Provide direct patient care, including administering medications, wound care, and treatments as prescribed by physicians

• Develop and implement individualized care plans, collaborating with interdisciplinary teams and healthcare professionals

• Monitor and document patient progress, reporting any changes or concerns to medical staff

• Maintain accurate and detailed records, adhering to HIPAA guidelines and facility policies

• Communicate effectively with patients, families, and healthcare team members to ensure seamless care transitions

• Participate in quality improvement initiatives, contributing to evidence-based practice and positive patient outcomes

Requirements:

• Currently licensed as a Licensed Practical Nurse (LPN) in the state of Arizona

• High school diploma or equivalent required; Associates degree or higher in Nursing preferred

• Minimum 1 year of experience in long-term care or post-acute care settings

• Ability to work effectively in a fast-paced environment, prioritizing patient needs and tasks

• Excellent communication, interpersonal, and organizational skills

• Ability to maintain patient confidentiality and adhere to HIPAA guidelines
