Clinical Coordinator - Clinic MA, LPN, RN

Overview We are seeking a Clinical Coordinator to assist with all clinical needs for Chapin clinic patients. The Clinical Coordinator plays a crucial role ensuring seamless coordination of patient care. This position involves proficient chart preparation, communication, scheduling of patient visits, appropriate utilization and logging of chronic care management activities, and provider and patient support. This position will be working in the Chapin clinic daily. This is a full-time, salary-based 8-hour position (8:00 AM–5:00 PM), Monday–Friday. Responsibilities Proficient chart preparation, communication, scheduling of patient visits, appropriate utilization and logging of chronic care management activities, and provider and patient support. Assess vital signs and provide hands-on patient support. Support clinic operations and patient care coordination in the Chapin clinic daily.
