Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)

Job Description

This assignment is located in a rural area, approximately 2 hours from Boston, MA. Candidates must have their own transportation as the facility has limited housing options and public transportation.

Key Responsibilities

• Continuously observe each resident for changes in mental and physical well-being, reporting pertinent observations to the Supervisor.
• Accurately receive, transcribe, and execute physician's orders.
• Administer medications and treatments as prescribed, ensuring proper documentation via a computerized system for MARs/TARs.
• Supervise nursing assistants in accordance with state licensing requirements, assigning and measuring resident care consistent with staff abilities.
• Accompany physicians during resident rounds to address medical needs and facilitate communication.
• Provide oxygen and administer gastrostomy/jejunostomy feedings as per facility procedures.
• Perform wound care and possess knowledge of wound vacs.
• Manage unit operations by checking appointments, monitoring therapy schedules, and overseeing documentation practices.
• Admit, discharge, and transfer residents while completing necessary documentation.
• Compile 24-hour reports for assigned units and maintain consistent communication with nursing management regarding resident issues.
• Communicate with families to resolve resident concerns and provide updates on new orders.
• Receive incoming calls and determine the necessary response.
• Assume responsibility for unit management during shifts under the direction of nursing leadership.
• Adhere to state and federal regulations concerning controlled medications, ensuring appropriate documentation and inventory management.
• Assist in the orientation and training of new nursing staff, recommending continuing education needs.

Qualifications

• Minimum of 3 years of relevant experience within the last 6 months.
• IV certification required or willingness to obtain during onboarding.
• COVID vaccination is mandatory, and declinations/exemptions are not accepted.
• Previous travel experience is highly preferred.
• Experience in a rural or small facility setting is highly preferred.

Additional Information

Candidates must be willing to float to different units based on experience and be available for every other weekend shifts. All requests for time off must be submitted for approval and cannot exceed five days, with holidays not being approved.
