Case Manager/Mental Health Counselor – Glenmore Manor The African American Planning Commission, Inc. (AAPCI) is a New York City-based nonprofit organization committed to reducing homelessness and addressing related issues such as domestic violence, HIV/AIDS, mental illness, substance abuse, shortage of affordable housing, and unemployment in the communities we serve. We build a team of professionals to service individuals and families with complex and chronic care needs to ensure they receive the right care, in the right setting and at the right time. Reporting Relationships: Case Manager/Mental Health Counselor reports to the Assistant Program Director. Principal Duties And Responsibilities Participate on the intake screening assessment team to determine whether tenant is suitable for independent living. Provides tenants with individualized mental health counseling, and co-leads specific resident population groups sessions. Use a Person-Centered approach to help tenants set goals, identify interests, and address concerns. Get to know tenants and their psychosocial history to provide strategic interventions and services. Provides education/information to adults and families and modifies service plan as necessary in response to tenant's progress. Participates in appropriate educational activities and department meetings. Directs adult tenants and/or families to appropriate community agencies and resources for financial assistance, counseling, rehabilitation, auxiliary care, or to aid in location of, e.g., medical supplies, adaptive devices. Prepares, writes, and maintains all required progress notes, reports, and group notes. Maintain a strengths-based perspective to engage all members of the family to help improve family functioning. Conduct visits in the tenant's unit to ensure compliance with lease and program rules. Develops and maintains contacts with various community agencies to gather information on available resources, to relay appropriate tenant information, and to improve services provided. Help tenants to understand and proactively address diagnoses (medical, psychiatric, substance abuse, etc.). Advocate and connect tenants to medical, mental health, and other supportive counseling services. Provide outreach in order to engage tenants who do not follow through with appointments and case management. Maintain a strengths-based perspective to engage all members of the family in order to help improve family functioning. Link tenants with vital community resources including medical, mental health, substance abuse treatment and support groups. Engage with community partners to provide services onsite and offsite. Conduct unit inspections in the tenant’s unit to ensure compliance with lease and program rules. Attends all internal trainings, development conferences and other mandated trainings scheduled by funding agency. Effectively guide tenants through the process of application, qualification, re-determination for SSI, SSDI, Medicaid, Medicare, HRA, HPD, SNAP, and other services as needed. Manages an assigned caseload of tenants ensuring comprehensive case management services are provided to both families and single adults. Performs other related duties as may be requested by immediate supervisor and in his/her absence the Program Director. Qualifications Minimal Qualifications Degree Requirement: Bachelor’s degree in a related field (social work, mental health counseling, psychology, sociology etc.) with at least 2-3 years’ experience in a residential setting. Other Qualifications Experience with one (1) or more populations including: mental health, substance use, inpatient/outpatient treatment, homelessness and supportive housing populations. Strong verbal and interpersonal communication skills with a focus on providing excellent tenant services. Demonstrated ability to interact effectively and collaboratively with a diverse community of residents, program staff and external partners. Ability to exercise good judgement and apply problem solving skills. Experience working collaboratively in a team oriented and outcomes focused environment. Ability to monitor and manage variety of tasks assigned. Compensation And Benefits Base salary range and benefits information for this position are being included in accordance with requirements of New York State and City pay transparency laws. Please note that salaries may vary for different individuals in the same role based on several factors, including but not limited to location of the role, individuals’ competencies, education/professional certifications, qualifications/experience and performance in the role. Base salary compensation range being offered for this role is $60,000.00 per year.
