Opportunity Details: Able to work evenings and weekends. Collaborates to provide care coordination activities in providing comprehensive care (e.g., other healthcare providers, local health departments, social service, nutritionist, dentist, community-based organizations, home care and patient education groups). Provide relevant health education and instructions, counsels patients/family responsible on the impact of illness and principles of health promotion and maintenance. Provides clinical and didactic instruction to professional staff, peers, students and community groups and conducts in-service educational programs, as needed. Participates in departmental meetings and interdisciplinary community committees, as well as peer review performance improvement meeting activities. Peer review. Refer to specialty services. Care gaps/Care gap metrics. Completion of assigned work ques in EPIC. Qualifications Board Eligible or Board Certified by the NCCPA Current NYS registration as a Physician Assistant Wages and Benefits include: Annual Base Salary: $131,127* - $174,836* based on 40-hour work week. The annual total value of compensation package is estimated at $144,239** - $192,319**, which includes the baseline salary, 401(k) contribution and other factors as set forth below: 401(k) Company Contribution (subject to IRS contribution limits): Employees are immediately vested in a 3% company contribution of base earnings. No employee match is required. After one year of service, employees receive an additional 7% company contribution of base earnings. No employee match is required. Annual Continuing Medical Education (CME) Reimbursement. Generous Annual Paid Time Off (PTO): Vacation, Sick, Holiday, and CME days.
