Direct Support Professional

Mentor Community Services , a part of the Sevita family, provides community‑based services for individuals with intellectual and developmental disabilities. Job Summary The Direct Support Professional (DSP) is a full‑time or part‑time, non‑exempt position paid hourly. The DSP supports 4 to 6 individuals in residential settings or up to 10 individuals in periodic programs, providing direct support or assistance in accordance with individual service or program plans. Responsibilities include socialization, health maintenance, medication administration, behavioral development, personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living. The DSP works 16 to 40 hours per week as scheduled, at the program location, and may accompany individuals into the community. Essential Job Functions Personnel Training: Attend orientation, ongoing training, and participate in monthly staff meetings. Employee Scheduling: Work scheduled hours, arrive on time, stay until replacement arrives, notify supervisor when unable to work, and fill in for other shifts without causing unapproved overtime. Teamwork: Work cooperatively with others, value contributions, be courteous, communicate openly, listen, and show respect. Workplace Safety: Comply with all safety policies and report all work‑related injuries to a supervisor. Services and Supports Direct Support: Assist with socialization, behavioral development, personal care, housekeeping, recreational activities, transportation, community orientation, shopping, financial management, citizenship, and other activities of daily living. Records: Accurately document progress and activity, review records and logs, maintain confidentiality, and complete billing documentation as applicable. Management of Individuals’ Assets: Assist with money management and ensure safety of individuals’ funds and property. Relationships: Maintain healthy, professional relationships with individuals, friends, families, guardians, and case managers; greet family members and visitors; implement Network Customer Service Standards. Reporting: Report instances of alleged abuse or neglect according to internal and external standards; report medical, behavioral, and other incidents per policy and requirements. Rights: Maintain confidentiality, respect the rights of persons served, practice universal precautions, and assist individuals in exercising their rights. Health Care Appointments: May accompany individuals to medical appointments and relay orders and information to and from medical providers. Medications: Accurately administer and document delivery of medications and treatments, promptly report administration errors, and maintain security of controlled medications and supplies. General Health Care: Monitor individuals’ health, document health concerns, communicate with a nurse or supervisor as appropriate, call 911 in emergencies, and follow individual health care directives. Medical Supplies and Equipment: Follow procedures for utilizing medical equipment and report malfunctions or repairs. Dietary Planning: Prepare or support individuals to prepare food, know special diets, prepare individual meals, monitor mealtime, and implement dining plans correctly. Maintenance Vehicles: Transport individuals safely, obey local laws, use safety equipment, and report accidents or safety concerns immediately. Housekeeping: Perform daily and seasonal housekeeping, maintain a clean environment, and report hazards. Maintenance and Repair: Monitor environmental safety, perform minor repairs, and report equipment or facility issues requiring repair. Safety: Check water temperature during bathing, participate in safety drills, and protect persons served during emergencies. Other Perform other related duties and activities as required. Employee may occasionally need to use a personal cell phone for business purposes and will receive a stipend to cover those costs. Minimum Knowledge and Skills Education and Experience High school diploma or equivalent preferred. Six months of human services experience preferred; must be 18 years of age. Certificates, Licenses, and Registrations Required Valid driver’s license in good standing; car registration and vehicle insurance if providing transportation. All state‑required training and certification completed within mandated timeframes. Blood Borne Pathogen Category Job requires duties that may expose the employee to blood, body fluids, or tissues; such exposure is an employment expectation. Other Skills and Abilities N/A Other Requirements Travel required as needed. Physical Requirements Heavy work: Exert up to 100 pounds occasionally, 50 pounds frequently, and 20 pounds constantly to move objects.
