LPN - Inpatient Case Management

Overview Sentara Norfolk General Hospital is hiring an LPN Intake Coordinator – Inpatient Case Management to work part‑time weekend day shift (20 hours per week) in Norfolk, VA. Responsibilities Triage incoming patients and verify medical information. Coordinate case management activities and liaise with physicians and other team members. Provide patient care support and ensure overall health needs are addressed. Requirements Licensed Practical Nurse, active in the state of Virginia. Minimum 1 year of clinical experience. Minimum 2 years of customer service experience. BLS certification required within 90 days of hire.
