Social Worker – Hospice

Shift: 7:00 AM - 7:00 PM

Assignment Details

• Contract Length: 5 weeks
• Guaranteed Hours: 40.00

Requirements

• Master's Degree in Social Work (required)
• One year experience in an acute or long-term medical setting (required)
• Two years' experience in an acute or long-term medical setting (preferred)
• Current BLS certification
• Current nursing license per the state of assignment
• Recent experience with EPIC within the last 12 months

Roles & Responsibilities

• Collaborate and facilitate patient discharge plans for individuals requiring post Hospice or post inpatient care.
• Assist patients, families, and caregivers in determining the appropriate level of Hospice care.
• Refer to outpatient resources as appropriate.
• Initiate and reassess plans of care according to the physical and emotional needs of the patient.
• Develop plans of care based on patient assessments, including psychosocial factors that may impact care.
• Perform other job-related duties as assigned.
