Senior Care Manager - Hospital Case Management & Discharge
South Mississippi Mental Health Center
South Mississippi Mental Health Center is seeking a Care Manager II to collaborate with multidisciplinary teams in managing patient care from admission through discharge. The role requires a graduate of an accredited nursing school (BSN preferred) or a Master's degree in Social Work, with two or more years of clinical experience, including one year in acute care.
